Private Fakehany and Lt. Frank Martin stepped inside a doorway in the Woolworth store with a hose line when the floor collapsed sending them some 15 feet to the basement below. Other firefighters attempted to rescue the duo, but were only able to pull Lt. Martin from the hole. Shortly after they went through the floor, a wall and portion of the floor next door also collapsed sliding into the basement where it trapped Fireman Fakehany by his legs. Fireman Fakehany was dead by the time firefighters reached him 45 minutes later.


James P. Fakehany was born February 4, 1914 and appointed to the Toledo Fire Department on July 17, 1940. He was assigned badge #405 and served at fire station 17.
